What is the name of this film?

I'm trying to remember a film I watched a long-long time ago. It is a
very psychedelic horror film. I remember some sort of perusing of this
bizarre jester like character in heavy make up. It was like a king but
looked like a jester of some sort.

The main part of the film I remember was this jester character pulling
out the intestines of someone using a clothing mangle.

It was quite old, very psychedelic and gruesome in parts. I can't
really remember much more than that which is a pain but I can't think
of the name and it's driving me crazy!!!
